Medical records is the main source for all student medical files dating back for at least 17 years. Student Health Services Medical Records can assist you with the following services:
- Requesting medical records for yourself
- Sending medical records to another provider or health facility outside of Student Health Services
If you are requesting medical records for yourself or another provider/health facility:
- you must first submit a Medical Release Form by mail, fax or walk in.
- When requesting records the new Red ID number is required. If you do not have a Red ID number please visit the official Red ID website and follow their directions for obtaining one.
- Processing your request will take 5-7 business days from the day of your request.
- There is a $1 per page fee (not to exceed $25). This fee does not apply to records requested for another provider/health facility.
- Please note that if the document is over 10 pages it CANNOT be faxed, only mailed or picked up in person.
- If it has been longer than 2 years since your last visit at Student Health Services please expect a longer wait time for processing your request.
